We are currently experiencing computer problems associated with the records request. The computer is currently out for repair. I apologize for any inconvenience, but I hope to rectify the situation shortly. Due to staffing issues, I have also changed the means of obtaining medical records.
To initiate a request, please send an email to firstname.lastname@example.org. Please attach the appropriate request form to the email. It MUST be correctly filled out. Each form will need the full name, address, and phone number for the receiving party. Each form will need a separate request for each child. Each form will need to be signed by the parent or guardian, including requests from medical facilities. Please note if the records have previously been sent to the parent or a medical facility, the parent must pay for a duplicate record before another one will be sent. To improve response time, records sent will consist only of immunization records and progress notes generated in this office. Records from specialists or hospitals will need to be obtained separately from the appropriate entity.
